综合性口腔护理干预方案在机械通气患儿中的应用

      目的 探究综合性口腔护理干预方案在机械通气患儿中的应用效果。方法 将2023年5月—12月泉州市妇幼保健院重症监护科接收的66例机械通气患儿选为研究对象。以密封信封抽签法分为综合干预组(n=33)及常规干预组(n=33)。常规干预组实施常规口腔护理,综合干预组实施综合性口腔护理模式干预。对比两组患儿口腔情况、并发症、静态全唾液流率(UWS)及机械通气时间。结果 干预后综合干预组Beck总分为(6.33±1.05)分,低于常规干预组的(8.55±1.85)分(t=6.069,P<0.001)。综合干预组出现呼吸机相关肺炎(VAP)发生率较常规干预组更低(3.03% vs 18.18%)(χ2=3.995,P=0.046)。干预后综合干预组UWS为(0.37±0.09)mg/min,高于常规干预组的(0.29±0.09)mg/min(t=3.611,P=0.001)。结论 综合性口腔护理模式能较大程度改进机械通气患儿的口腔卫生问题,对菌斑、口腔异味及牙龈情况均有改善作用;也能减少并发症发生率,提升UWS,维持口腔卫生健康。

      Objective To explore the effect of comprehensive oral nursing intervention in children with mechanical ventilation.Methods A total of 66 mechanically ventilated children admitted to the intensive care department of the hospital from May 2023 to December 2023 were selected as the study objects.They were divided into comprehensive intervention group(n=33)and routine intervention group(n=33)by sealed envelope lottery method.The routine intervention group implemented routine oral care,and the comprehensive intervention group implemented comprehensive oral care mode intervention.Oral conditions,complications,unstimulated whole saliva(UWS)and mechanical ventilation time were compared between the two groups.Results After the intervention,the total Beck score of the comprehensive intervention group was(6.33±1.05),which was lower than that of the conventional intervention group([8.55±1.85],t=6.069,P<0.001).The incidence of ventilator-associated pneumonia(VAP)in the comprehensive intervention group was lower than that in the conventional intervention group(3.03% vs 18.18%)(χ2=3.995,P=0.046).After the intervention,the UWS of the comprehensive intervention group was(0.37±0.09)mg/min,which was higher than that of the conventional intervention group([0.29±0.09] mg/min,t=3.611,P=0.001).Conclusions The comprehensive oral care mode can improve the oral health of children with mechanical ventilation to a great extent,and can improve the plaque,oral odor and gingiva condition.It can also reduce the incidence of complications,improve UWS,and maintain oral health.

    Autosomal dominant polycystic kidney disease(ADPKD)stands as a leading cause of inherited renal failure,with current therapeutic strategies lacking effective means to reverse disease progression.Stem cell therapy,owing to its potential in tissue repair and immunomodulation,has emerged as a focal point of research.This review systematically elucidates the mechanisms by which hematopoietic stem cells(HSCs)intervene in ADPKD pathology through paracrine effects,metabolic reprogramming,and immune regulation.Furthermore,it summarizes clinical translational evidence of HSCs in both acute kidney injury and chronic kidney disease.This article also comparatively analyzes the unique advantages and challenges of mesenchymal stem cells(MSCs)and induced pluripotent stem cells(iPSCs)in the context of ADPKD treatment,thereby providing theoretical support for the development of multimodal stem cell therapies.

       癫痫是一种由多种病因引起的慢性脑部疾病,其患者死亡风险为一般人群的2~3倍。我国农村地区,受多种因素的影响,约2/3的癫痫患者未得到合理的治疗,给患者及其家庭和社会带来沉重负担。顺德区积极响应“中国农村地区癫痫防治管理项目”,成立癫痫专科医联体,构建三级诊疗体系,开展了一系列实践探索。本文旨在总结顺德区专科医联体管理模式下的癫痫综合管理实践,分析管理现状,并提出改进路径,以期为癫痫患者提供更优质的服务,改善患者生活质量。

      Epilepsy is a chronic brain disorder caused by various etiologies,the mortality risk of epilepsy patients is 2 to 3 times higher than that of the general population.In rural areas of China,due to multiple influencing factors,approximately two-thirds of epilepsy patients receive inadequate or no proper treatment,imposing a heavy burden on patients,their families,and the society.Shunde District has actively responded to the “China Rural Epilepsy Prevention and Management Project” by establishing an epilepsy specialty medical consortium and building a three-tiered diagnostic and treatment system,conducting a series of practical explorations.This article aims to summarize the comprehensive epilepsy management practices under the specialty medical consortium model in Shunde District,analyze the current management status,and propose improvement strategies to provide better services for epilepsy patients and enhance their quality of life.

运用信息技术完善医用耗材监督管理的研究——供应链溢出视角

在按病种分值付费(DIP)模式深化实施的背景下,医用耗材管理作为医院成本控制与医疗质量保障的核心环节,其监督效能直接影响医院运营效益。本文基于供应链溢出视角,以某医院医用耗材信息化管理实践为案例,从技术溢出、管理溢出、风险溢出三个维度,探讨信息技术对医院内部医用耗材监督管理的优化路径。研究表明,通过构建医疗器械唯一标识(UDI)系统、收拾耗占比动态监控、专项议价管控等措施,可实现监督技术跨环节渗透、管理标准全链条协同、风险防控全周期覆盖。实践表明,该模式有效控制了医院耗材的整体运营成本,并显著提升了高值耗材的用后结算管理水平,为公立医院构建精准、高效的内部监管机制提供了有价值的实践路径。

Under the deepening implementation of the diagnosis-intervention packet(DIP) payment model,the management of medical consumables serves as a core aspect of hospital cost control and medical quality assurance,with its supervision efficiency directly impacting hospital operational benefits.Based on the perspective of supply chain spillover,this paper takes the informatization management practices of medical consumables in a hospital as a case study,exploring the optimization paths of information technology in the internal supervision and management of medical consumables from three dimensions:technological spillover,managerial spillover,and risk spillover.The research shows that by establishing a unique device identification(UDI) system,implementing dynamic monitoring of the consumption ratio of medical consumables,and conducting special negotiation controls,cross-process penetration of supervision technology,full-chain coordination of management standards,and comprehensive risk prevention and control throughout the entire cycle can be achieved.Practice demonstrates that this model has reduced the hospital’s overall cost of medical consumables by 2.75%,increased the post-use settlement rate of high-value consumables to 80%,and provided practical references for public hospitals to establish a precise and efficient internal supervision system.

       乳腺癌(BC)发病率持续上升,已成为严重威胁公共健康的重大挑战,转移仍是导致患者死亡的主要原因。目前,切除肿瘤并辅以放化疗后仍有较大几率发生转移。此外,早期转移隐匿性强,干预措施不完善及耐药发生迅速均是预后不良的重要因素。细胞外囊泡(EVs)是由细胞释放的、具有膜结构的纳米级颗粒,是细胞间通讯的重要介质。EVs通过转运膜性组分、胞质蛋白及核酸等,调控乳腺癌的发生、进展及转移定植,其来源异质性决定了其在乳腺癌转移中发挥独特作用。本文重点总结了不同来源EVs作为液体活检标志物在转移预警中的价值,以及靶向EVs分泌或阻断其通讯网络在克服免疫治疗耐药中的潜在临床应用,以期为临床医生提供有价值的诊疗参考。
     The incidence of breast cancer(BC) continues to rise and has become a major public health challenge.Metastasis remains the principal cause of BC-related mortality.Currently,there remains a high probability of metastasis even after surgical tumor resection followed by radiotherapy and chemotherapy.The insidious nature of early metastasis,inadequate intervention strategies,and rapid development of drug resistance are all critical factors contributing to poor prognosis.Extracellular vesicles(EVs) are membrane-enclosed,nanoscale particles released by cells.As mediators of intercellular communication,EVs transfer membrane components,cytosolic proteins,and nucleic acids to recipient cells,thereby modulating BC initiation,progression,and metastatic dissemination.The heterogeneity of EV origins determines their distinct roles in breast cancer metastasis.This review focuses on summarizing the value of EVs from different sources as liquid biopsy biomarkers for metastasis warning,as well as their potential clinical applications in targeting EV secretion or blocking their communication networks to overcome immunotherapy resistance,aiming to provide valuable diagnostic and therapeutic references for clinicians.

肺结核合并Ⅱ型呼吸衰竭患者经鼻高流量湿化氧疗期间应用多维度强化护理对疗效及气道重塑的影响

      目的 分析肺结核合并Ⅱ型呼吸衰竭患者经鼻高流量湿化氧疗期间应用多维度强化护理的效果。方法 纳入2022年12月—2025年1月在河北省胸科医院进行经鼻高流量湿化氧疗的肺结核合并Ⅱ型呼吸衰竭患者294例,随机分为强化护理组(147例,多维度强化护理)、对照组(147例,常规护理),测量比较两组患者在护理前后气道重塑指标、肺功能指标、血气分析指标,进行疗效评估。结果 护理后强化护理组气道壁面积、气道壁厚度低于对照组,气道腔面积高于对照组(P<0.05);护理后强化护理组第1秒用力呼气容积、用力肺活量高于对照组(P<0.05);护理后强化护理组动脉血氧分压高于对照组,动脉血二氧化碳分压低于对照组(P<0.05);强化护理组总有效率高于对照组(P<0.05)。结论 肺结核合并Ⅱ型呼吸衰竭患者经鼻高流量湿化氧疗期间接受多维度强化护理可改善其气道重塑及肺功能,有利于血气分析指标恢复,应用效果显著。

       Objective To analyze the efficacy of multidimensional enhanced nursing during nasal high-flow humidified oxygen therapy in patients with pulmonary tuberculosis and type Ⅱ respiratory failure.Methods A total of 294 patients with pulmonary tuberculosis and type Ⅱ respiratory failure who received nasal high-flow humidified oxygen therapy at Hebei Chest Hospital between December 2022 and January 2025 were enrolled and randomly divided into an enhanced nursing group(147 patients,receiving multidimensional enhanced nursing)and a control group(147 patients,receiving conventional nursing).Airway remodeling,pulmonary function,and blood gas analysis were measured and compared between the two groups before and after nursing,and the efficacy was evaluated.Results After nursing,the enhanced nursing group had lower airway wall area and airway wall thickness,and higher airway lumen area than the control group(P<0.05).The forced expiratory volume in 1 second and forced vital capacity in the enhanced nursing group were higher than those in the control group(P<0.05).The blood oxygen partial pressure(PaO2)in the enhanced nursing group was higher than that in the control group,and the carbon dioxide partial pressure(PaCO2)was lower than that in the control group(P<0.05).The total effective rate in the enhanced nursing group was higher than that in the control group(P<0.05).Conclusions Multidimensional enhanced nursing during nasal high-flow humidified oxygen therapy can improve airway remodeling and lung function in patients with pulmonary tuberculosis and type Ⅱ respiratory failure,and is beneficial for the recovery of blood gas analysis indicators.The application effect is significant.

      Gegen Qinlian decoction(GQD),as a traditional Chinese herbal formula,has demonstrated significant efficacy in treating metabolic diseases.With advancements in modern research methodologies for Chinese herbal medicine,the therapeutic effects and molecular mechanisms of GQD in metabolic diseases have been progressively uncovered,including the anti-inflammatory properties of its active components such as puerarin,baicalin,and berberine.However,emerging theories and mechanisms in the field of herbal research,such as modulation of gut microbiota,production of plant-derived extracellular vesicles,and self-assembly of components in herbal decoctions,continuously refine our understanding,highlighting the need to update insights into GQD’s mechanisms of action.This review aims to summarize the roles and mechanisms of GQD in metabolic diseases,integrating the latest research progress with a focus on findings related to gut microbiota,plant-derived extracellular vesicles,and self-assembly phenomena.It further seeks to explore potential therapeutic targets and future research directions,thereby providing guidance for clinical applications and fundamental studies.

早产儿母亲育儿胜任感的多路径作用机制:基于结构方程模型的验证

Multi-pathway mechanism of parenting competence in premature infant mothers:Validation based on structural equation modeling

       目的   基于结构方程模型(SEM)验证早产儿母亲育儿胜任感的多路径作用机制。方法  采用便利抽样法选取2024年6月—2025年6月在莆田学院附属医院分娩的早产儿母亲250例作为研究对象。采用一般资料调查表、中文版育儿胜任感量表(C-PSOC)、婴儿母亲育儿支持问卷(PSM)、角色适应问卷、简式亲职压力量表收集数据。通过单因素分析及多元线性回归分析母亲育儿胜任感的影响因素,使用AMOS软件构建结构方程模型,分析早产儿分娩后母亲育儿胜任感的作用路径。结果  250例早产儿母亲的C-PSOC得分为(61.93±6.02)分,多元线性回归分析结果显示,早产儿母亲育儿胜任感的影响因素包括产次、育儿支持、角色适应、亲职压力(均P<0.05)。结构方程模型拟合良好(χ 2 /df=1.026,GFI=0.987,AGFI=0.978,NFI=0.987,CFI=1.000,RMSEA=0.010),其中角色适应正向预测育儿胜任感(β=0.344),育儿支持(β=-0.477)与亲职压力(β=-0.283)负向预测(均P<0.05),并且角色适应通过育儿支持、亲职压力间接提升育儿胜任感(效应值0.467);产次经角色适应间接降低压力源影响(效应值0.529)。结论  早产儿母亲育儿胜任感受多路径机制调控,临床需针对角色适应、育儿支持及亲职压力设计级联干预策略。
       Objective  To verify the multi-pathway mechanism of parenting competence of premature infant mothers based on structural equation modeling(SEM).Methods  A convenience sampling method was used to select 250 mothers of preterm infants who delivered in Affiliated Hospital of Putian University between June 2024 and June 2025 as the study subjects.Data was collected using a general information survey,the Chinese version of the Parenting Sence of Competence Scale(C-PSOC),the Parenting Support Questionnaire for Infant Mothers(PSM),the Role Adaptation Questionnaire,and the Simplified Parenting Stress Scale.By conducting  single factor analysis and multiple linear  regression analysis on the influencing factors of maternal parenting competence,a structural equation model was constructed using AMOS software to analyze the pathway of maternal parenting competence after premature birth.Results  The C-PSOC score of 250 mothers of premature infants was(61.93±6.02).Multiple linear  regression analysis showed that the influencing factors of parenting competence among mothers of premature infants included parity,parenting support,role adaptation,and parental pressure(all P<0.05).The structural equation model fits well(2/df=1.026,GFI=0.987,AGFI=0.978,NFI=0.987,CFI=1.000,RMSEA=0.010),which  role adaptation  positively  predicted parenting competence(β=0.344),parenting support(β=-0.477)and parenting stress(β=-0.283)negatively predicted(all P<0.05),and role adaptation indirectly enhanced parenting competence through parenting support and parenting stress(effect value 0.467).The adaptation of roles during childbirth indirectly reduced the impact of stressors(effect value 0.529).Conclusions  The multi-pathway mechanism of parental competence perception regulation in premature infant mothers requires the design of cascading intervention strategies targeting role adaptation,parenting support,and parental stress in clinical practice.

       纤毛是细胞表面的重要细胞器,广泛参与细胞运动、感知外界信号和维持器官功能等生理过程。纤毛的形成,即纤毛发生(ciliogenesis)是一个高度复杂且受精密调控的过程,涉及大量与纤毛结构和功能相关基因的表达与调控。近年来,随着基因组学和发育生物学的发展,越来越多的研究揭示了多种关键转录因子在纤毛发生中的调控作用,包括RFX家族、FOXJ1、MCIDAS、GEMC1、MYB、E2F等。这些转录因子共同构成了一个多层次、多通路交织的调控网络,调控纤毛组装、基体复制、纤毛定位和功能维持等多个方面。本文系统综述了纤毛相关基因转录调控的研究进展,特别是关键转录因子的功能、相互作用及其在纤毛病中的作用,为深入理解纤毛的发育机制和疾病治疗提供参考。
       Cilia are crucial cell-surface organelles involved in cell movement,signal sensing,and organ function maintenance.Their formation,or ciliogenesis,is a complex and  precisely controlled  process that  requires the expression and regulation of numerous cilia-related genes.Recent advances in genomics and developmental biology have uncovered the  regulatory roles of key transcription factors like the RFX family,FOXJ1,MCIDAS,GEMC1,MYB,and E2F in ciliogenesis.These factors form a multi-level,interconnected regulatory network that oversees cilium assembly,basal body replication,ciliary positioning,and function preservation.This review systematically examines current research on transcriptional regulation of ciliary genes,with a focus on the roles,interactions,and contributions of these key transcription factors to ciliopathies,offering insights into ciliary development and disease treatment.

基于思维导图的绿色通道急救干预对急性心梗患者介入治疗的效果

The effect of green channel emergency nursing based on mind mapping on PCI of acute myocardial infarction patients

       目的   探讨基于思维导图的绿色通道急救干预对急性心肌梗死患者介入治疗的效果。方法   对天津市人民医院2022年1月—2024年12月收治的80例AMI患者进行研究,依照收治时间进行分组,将2022年1月—2023年6月采取常规急救流程干预的40例患者分为常规组,将2023年7月—2024年12月的采取基于思维导图的绿色通道急救干预的40例患者分为思维导图组。比较两组患者急救效率、急救前后生命体征(心率、舒张压、收缩压)、心肌损伤指标[磷酸肌酸激酶(CK-MB)及肌钙蛋白Ⅰ(cTnI)],最后比较两组患者干预满意度。结果   对比急救效率发现,思维导图组的预检时间、会诊时间、导管室激活时间、急诊送至PCI手术时间均短于常规组(P<0.05);对比急救前后生命体征发现,与急救前对比,急救后两组的心率、舒张压、收缩压均降低(P<0.05),急救前后两组心率、舒张压、收缩压比较差异无统计学意义(P>0.05);对比急救前后心肌损伤指标发现,急救前两组CK-MB、cTnI比较差异无统计学意义(P>0.05),急救后,思维导图组CK-MB、cTnI低于常规组与急救前(P<0.05);对比干预满意度发现,思维导图组干预满意度为100.00%,高于常规组的85.00%(P<0.05)。结论  基于思维导图的绿色通道急救干预可提升AMI患者的急救效率,稳定患者生命体征,减轻心肌损伤的同时,提升患者干预满意度。
       Objective  To explore the effect of green channel emergency nursing based on mind mapping on percutaneous coronary intervention(PCI) of patients with acute myocardial infarction(AMI).Methods  A study was conducted on 80 AMI patients admitted to Tianjin People’s Hospital from January 2022 to December 2024.The patients were divided into two groups based on their admission time.The 40 patients who received routine emergency nursing interventions from January 2021 to June 2023 were assigned to the conventional group,while the 40 patients who  received green channel emergency nursing based on mind mapping from July 2023 to November 2024 were assigned to the mind mapping group.The emergency treatment efficacy,vital signs(heart rate,diastolic blood pressure,systolic bloodpressure),myocardial injury indicators(phosphocreatine kinase[CK-MB],and troponin I[cTn1])between two groups of patients before and after emergency treatment were compared.Results  The pre-examination time,consultation time,catheterization room activation time and emergency room to PCI operation time of mind mapping group were all shorter than those of conventional group(P<0.05).Compared with the vital signs before and after emergency treatment,we found that the heart rate,diastolic pressure and systolic blood pressure of the two groups decreased after emergency treatment(P<0.05),and there was no significant difference between the heart rate,diastolic pressure and systolic blood pressure of the two groups before and after emergency treatment(P>0.05).Comparing the myocardial injury indicators before and after emergency treatment,there was no significant difference in CK-MB and cTn1 between the two groups before emergency treatmentP>0.05).After emergency treatment,CK-MB and cTn1 in the mind map group were lower than those in the conventional group and before emergency treatment(P<0.05).Comparing the intervention satisfaction,it was found that the mind mapping group of 100.00% was higher than the conventional group of 85.00%(P<0.05).Conclusions  Green channel emergency nursing based on mind mapping can improve the efficiency of emergency treatment for AMI patients,stabilize their vital signs,reduce myocardial injury,improve patient intervention satisfaction.
