目的 探讨在分化型甲状腺癌(DTC)患者的治疗中,采用早期积极心理进行干预,对患者负性情绪和癌因性疲乏的影响。方法 抽取2021年6月—2023年1月平煤神马医疗集团总医院收治的160例DTC癌患者,随机分为对照组(常规护理)和研究组(常规护理+早期积极心理干预),每组各80例,评估患者干预前后的心理状态、癌因性疲乏、生活质量、护理工作满意度。结果 护理干预1~4周后,两组患者汉密尔顿抑郁量表(HAMD)评分、汉密尔顿焦虑量表(HAMA)评分均降低,并且研究组患者评分更低(P<0.05);干预后,两组患者的癌因性疲乏评分均升高,但是研究组患者的以上指标升高幅度小于对照组(P<0.05);此外,两组患者的护士观察量表(NOSIE)评分均明显改善,并且研究组变化幅度大于对照组(P<0.05);研究组患者的护理满意度高于对照组(95.00% vs 82.50%,P<0.05)。结论 对DTC患者开展早期积极心理干预,能够帮助患者改善负性情绪,减轻癌因性疲乏,提高患者的生活质量和护理满意度。
Objective To investigate the effect of early positive psychological intervention on negative emotions and cancer-related fatigue in the treatment of differentiated thyroid cancer(DTC) patients.Methods From June 2021 to January 2023,160 patients with DTC admitted to General Hospital of Pingmei Shenma Group were randomly divided into the control group(routine nursing)and the research group(routine nursing + early positive psychological intervention),with 80 patients in each group.The psychological state,cancer-related fatigue,quality of life and nursing satisfaction of patients before and after intervention were evaluated.Results After 1~4 weeks of intervention,the scores of Hamilton Depression Scale(HAMD)and Hamilton Anxiety Scale(HAMA)were decreased in both groups,and the scores of research group were lower(P<0.05).After intervention,the cancer-related fatigue scores of both groups were increased,but the increase of research group were less than that of control group(P<0.05).Moreover,NOSIE scores of both groups were significantly improved,and the change range of research group was greater than that of control group(P<0.05).The nursing satisfaction of research group was higher than that of control group(95.00% vs 82.50%,P<0.05).Conclusions Early positive psychological intervention for patients with DTC can effectively improve negative emotions,reduce cancer-related fatigue,and improve nursing satisfaction of patients.
目的 探讨乳腺癌术后疼痛综合征(PMPS)的发生率及影响因素。方法 选择2021年1月—2023年2月医院收治的82例乳腺癌患者为研究对象,采取手术治疗,统计PMPS发生率,分析PMPS的特征,比较PMPS患者和非PMPS患者的临床资料,采用 Logistic 逐步回归分析。结果 82例中有20例患者术后发生PMPS,发生率为24.39%,其中患侧腋窝45.00%、麻木样疼痛35.00%、中度疼痛60.00%、术后即刻疼痛50.00%、每日发作疼痛50.00%占比较高。PMPS患者和非PMPS患者的体质指数、教育程度、病理分期、肿瘤占位、手术方式、术前使用非甾体抗炎药、术前化学治疗、术后化学治疗比较差异均无统计学意义(P>0.05)。PMPS患者年龄低于非PMPS患者,负性情绪率30.00%高于非PMPS患者8.06%,清扫腋窝淋巴结率95.00%高于非PMPS患者72.58%,术后放射治疗率30.00%高于非PMPS患者6.45%(P<0.05)。年龄、负性情绪、清扫腋窝淋巴结、术后放射疗为PMPS发生的危险因素(P<0.05)。结论 乳腺癌术后较容易发生PMPS,主要表现为术后即刻腋窝疼痛、麻木,发作频率较高,疼痛较重,其中年龄小、术前焦虑、清扫腋窝淋巴结、术后放疗为引发PMPS的危险因素,需加强监测和针对性处理,研究价值较高。
Objective To investigate the incidence and influencing factors of post-mastectomy pain syndrome(PMPS)in breast cancer patients.Methods A total of 82 patients with breast cancer admitted to our hospital from January 2021 to February 2023 were selected as the research objects,and underwent surgical treatment.The incidence of PMPS was counted,the characteristics of PMPS were analyzed,and the clinical data of PMPS patients and non-PMPS patients were compared.Results Among the 82 patients,20 patients had PMPS after surgery,with an incidence of 24.39%.Among them,the affected axilla accounted for 45.00%,numbness pain 35.00%,moderate pain 60.00%,immediate postoperative pain 50.00%,and daily pain 50.00%.There were no significant differences in body mass index,education level,pathological stage of disease,tumor location,surgical method,preoperative use of non-steroid anti-inflammatory drugs,preoperative chemotherapy and postoperative chemotherapy between PMPS patients and non-PMPS patients(P>0.05).The age of PMPS patients was significantly higher than that of non-PMPS patients,the rate of negative emotion was 30.00%,the rate of axillary lymph node dissection was 95.00%,and the rate of postoperative radiotherapy was 30.00%,which was significantly higher than that of non-PMPS patients(P<0.05).Age,negative emotion,axillary lymph node dissection and postoperative radiotherapy were independent risk factors for PMPS(P<0.05).Conclusions PMPS is prone to occur after breast cancer surgery,mainly characterized by immediate postoperative axillary pain and numbness,with a high frequency and severe pain.Young age,preoperative anxiety,axillary lymph node dissection,and postoperative radiotherapy are independent risk factors for PMPS,which need to be strengthened monitoring and targeted treatment.
目的 探讨微RNA-29b(miR-29b)对子宫内膜癌细胞增殖、迁移和侵袭的影响。方法 子宫内膜癌HEC-1-B细胞分为miR-29b模拟物组(MM组)、miR-29b阻遏物组(MR组)和阴性对照物组(MNC组),分别转染miR-29b拟似物、miR-29b阻遏物和miR-29b阴性对照物,每组设置6个复孔。以实时定量逆转录PCR检测miR-29b表达,以水溶性四氮唑(WST-1)检测miR-29b对HEC-1-B子宫内膜癌细胞增殖的影响,以Transwell小室检测HEC-1-B子宫内膜癌细胞迁移和侵袭的影响,以Western blot法检测磷酸酶张力蛋白同源物(PTEN)-蛋白激酶 B(AKT)通路蛋白表达水平。结果 MNC组、MM组、MR组miR-29b相对表达量分别为(2 032.1±873.4)、(19 272.8±2 087.9)、(472.7±105.6),组间比较差异有统计学意义(P<0.05)。MM组0、3、5、7 d时OD值分别为(0.32±0.06)、(0.53±0.08)、(1.13±0.12)和(1.92±0.14),MNC组0、3、5、7 d时OD值分别为(0.34±0.09)、(0.71±0.08)、(1.67±0.21)和(3.49±0.24),MR组0、3、5、7 d时OD值分别为(0.38±0.09)、(0.84±0.18)、(2.43±0.24)和(5.67±0.15),3组0 d时OD值比较差异无统计学意义(P=0.216),三组3 d、5 d、7 d时OD值比较差异存在统计学意义(P<0.001)。MNC组、MM组和MR组迁移细胞数分别为(403.9±23.8)(102.6±15.7)和(685.7±46.8)个,上述3组侵袭细胞数分别为(82.1±12.7)(38.2±10.6)和(124.6±21.6)个,MM组和MNC组上述指标比较差异均有统计学意义(P<0.05),MR组和MM组上述指标比较差异均有统计学意义(P<0.05)。MNC组、MM组、MR组PTEN蛋白相对表达量分别为(0.25±0.08)、(0.69±0.11)、(0.11±0.05),上述3组p-AKT蛋白相对表达量分别为(0.58±0.10)、(0.13±0.06)和(0.79±0.08),上述3组AKT蛋白相对表达量分别为(0.38±0.09)、(0.37±0.11)和(0.37±0.08),MM组与MNC组PTEN、p-AKT水平比较差异有统计学意义(P<0.05),AKT水平比较差异无统计学意义(P>0.05);MR组与MNC组、MM组PTEN、p-AKT水平比较差异有统计学意义(P<0.05),AKT水平比较差异无统计学意义(P>0.05)。结论 过表达miR-29b对子宫内膜癌细胞增殖、迁移和侵袭具有抑制作用,靶向PTEN-AKT可能是其重要作用途径。
Objective To investigate the effects of microRNA-29b on proliferation,migration and invasion of endometrial cancer cells.Methods The endometrial cancer HEC-1-B cells were divided into micro29b mimetic group(MM group),micro29b repressor group(MR group)and negative control group(MNC group),and the micro29b mimetic,micro29b repressor and micro29b negative control were transfected into each group,six compound holes with each group.The real-time quantitative reverse transcription polymerase chain reaction(RT-PCR)was used to detect the expression of mi29b,WST-1 was used to detect the effect of mi29b on the proliferation of HEC-1-B endometrial cancer cells,Transwell chamber was used to detect the migration and invasion of HEC-1-B endometrial cancer cells,and Western blot was used to detect the expression level of PTEN-AKT pathway protein.Results The relative expression levels of microRNA-29b in MNC group,MM group and MR group were(2 032.1±873.4),(19 272.8±2 087.9)and(472.7±105.6),respectively,and there were significant differences between groups(P<0.05).OD values of MM group at 0 d,3 d,5 d and 7 d were(0.32±0.06),(0.53±0.08),(1.13±0.12)and(1.92±0.14)respectively.The OD values of MNC group at 0,3,5 and 7 days were(0.34±0.09),(0.71±0.08),(1.67±0.21)and(3.49±0.24)respectively.The OD values of MR group at 0 d,3 d,5 d and 7 d were(0.38±0.09),(0.84±0.18),(2.43±0.24)and(5.67±0.15)respectively.There was no significant difference in OD value between the three groups on day 0 (P=0.216).There were significant differences in OD value between the three groups on day 3,day 5 and day 7(P<0.001).The number of migrating cells in MNC group,MM group and MR group were(403.9±23.8)cells,(102.6±15.7)cells and(685.7±46.8)cells,respectively.The number of invasive cells in the above three groups were(82.1±12.7)cells,(38.2±10.6)cells and(124.6±21.6)cells.There were significant differences in the above indexes between MM group and MNC group(P<0.05),also between MR group and MM group(P<0.05).The relative expression levels of PTEN protein in MNC group,MM group and MR group were(0.25±0.08),(0.69±0.11)and(0.11±0.05).The relative expression levels of p-AKT protein in the above three groups were(0.58±0.10),(0.13±0.06)and(0.79±0.08).The relative expression levels of AKT protein in the above three groups were(0.38±0.09),(0.37±0.11)and(0.37±0.08),respectively.Compared with MNC group,the levels of PTEN and p-AKT in MM group had statistical significance(P<0.05),but there was no statistical difference in AKT level(P>0.05).Compared with MNC group and MM group,the levels of PTEN and p-AKT in MR group had statistical significance(P<0.05),and there was no statistical difference in AKT level(P>0.05).Conclusions Overexpression of microRNA-29b can inhibit the proliferation,migration and invasion of endometrial cancer cells,and targeting PTEN-AKT may be an important pathway.
目的 探讨80例局部宫颈癌根治性同步放化疗的临床疗效及预后影响因素。方法 选取鹤壁市妇幼保健院2018年1月—2021年1月收治的80例宫颈癌患者进行回顾性分析,患者依照其病变程度均采取积极的手术与同步放化疗,其中40例患者采取单纯化疗,将其分为化疗组,40例患者采取同步放化疗,将其分为同步放化疗组,分析其近远期临床疗效与不良反应发生率。对所有患者进行3年随访,将患者分为两个亚组,即预后不良组(n=20)和预后良好组(n=60),对比两组患者一般临床特征,应用Logistic回归模型分析局部宫颈癌根治性同步放化疗的预后影响因素。结果 同步放化疗组ORR、DCR高与化疗组(P<0.05),对照组中位无进展生存期为5.4(2.38~14.52)个月。观察组中位无进展生存期为6.66(2~20.1)个月,观察组高于对照组(χ2=4.536,P=0.041);同步放化疗组盆腔积液、阴道炎症、泌尿生殖道反应、直肠反应、骨髓抑制、胃肠道反应发生率略高于化疗组,但两组对比差异无统计学意义(P>0.05);预后良好组与预后不良组患者年龄、是否绝经、病理类型、肿瘤大小对比差异无统计学意义(P>0.05),预后良好组与预后不良组患者临床分期、组织分化程度、淋巴结转移、是否同步放化疗、治疗前血红蛋白水平对比差异有统计学意义(P<0.05);组织分化程度低、未同步放化疗、治疗前血红蛋白水平低为局部宫颈癌的预后不良影响因素(P<0.05)。结论 对局部宫颈癌患者采取根治性同步放化疗与单一化疗相比可提升其临床疗效与远期生存率,同时安全性较高。组织分化程度低、未同步放化疗、治疗前血红蛋白水平低为宫颈癌预后不良影响因素。
Objective To explore the clinical efficacy and prognostic factors of 80 cases of local cervical cancer treated with radical synchronous radiotherapy and chemotherapy.Methods A retrospective analysis was conducted on 80 cervical cancer patients admitted to Hebi Maternal and Child Health Hospital from January 2018 to January 2021.Patients underwent surgery and synchronous radiotherapy and chemotherapy according to their degree of lesion.Among them,40 patients received simple chemotherapy and were divided into a chemotherapy group,while 40 patients received synchronous radiotherapy and chemotherapy and were divided into a synchronous radiotherapy and chemotherapy group.The short-term and long-term clinical efficacy and incidence of adverse reactions were analyzed.A 3-year follow-up was conducted on all patients,and patients were divided into two subgroups,namely the poor prognosis group(n=20)and the good prognosis group(n=60).The general clinical characteristics of the two groups of patients were compared,and a Logistic regression model was used to analyze the prognostic factors of local cervical cancer radical synchronous radiotherapy and chemotherapy.Results The objective relief rate and disease control rate of the synchronous radiotherapy and chemotherapy group were significantly higher than those of the chemotherapy group(P<0.05),and the median progression free survival of the control group was 5.4(2.38-14.52)months.The median progression free survival of the observation group was 6.66(2-20.1)months,which was higher than that of the control group(χ2=4.536,P=0.041).The incidence of pelvic fluid accumulation,vaginitis,urogenital reactions,rectal reactions,bone marrow suppression,and gastrointestinal reactions in the synchronous radiotherapy and chemotherapy group was slightly higher than that in the chemotherapy group,but there was no statistically significant difference between the two groups(P>0.05).There was no statistically significant difference in age,menopause,pathological type,and tumor size between the patients with good prognosis and those with poor prognosis(P>0.05).However,there was a statistically significant difference in clinical stage,tissue differentiation,lymph node metastasis,synchronous radiotherapy and chemotherapy,and pre-treatment hemoglobin levels between the patients with good prognosis and those with poor prognosis(P<0.05).Low degree of tissue differentiation,lack of synchronous radiotherapy and chemotherapy,and low hemoglobin levels before treatment were adverse prognostic factors for local cervical cancer(P<0.05).Conclusion sCompared with single radiotherapy,radical synchronous radiotherapy and chemotherapy can improve the clinical efficacy and long-term survival rate of patients with local cervical cancer,with higher safety.The severe tissue differentiation,unsynchronized chemoradiotherapy and hemoglobin before treatment were the adverse prognostic factors of cervical cancer.
目的 探讨无痛消化内镜下高频电刀治疗对胃癌前病变的应用效果及复发率的影响。方法 选取甘肃医学院附属医院2017年1月—2020年1月收治的120例胃癌前病变患者进行回顾性分析,依照其手术治疗方案将其分为两组,各60例。对照组采取常规内镜黏膜下剥离术,观察组患者采取无痛消化内镜下高频电刀剥离治疗。对比两组的切除速度、切除面积、治愈性切除率、完全及整块切除率,对患者进行3年随访,记录其复发情况,并对比两组患者并发症情况。结果 两组的切除面积、治愈性切除率、完全切除率及整块切除率对比差异无统计学意义(P>0.05),观察组切除速度为(27.34±8.58)mm2/min,高于对照组的(15.54±4.52)mm2/min,差异有统计学意义(P<0.05)。两组患者术后3年的总复发率(28.33% vs 30.00%)对比差异无统计学意义(P>0.05)。观察组感染、穿孔等相关并发症发生率为3.33%,低于对照组的15.00%(P<0.05)。结论 无痛消化内镜下高频电刀与常规内镜黏膜下剥离术治疗胃癌前病变相比具有显著疗效。但无痛消化内镜下高频电刀治疗可提升切除速度,降低并发症发生率。
Objective To explore the application effect and recurrence rate of painless endoscopic high-frequency electrocautery on gastric precancerous lesions.Methods A total of 120 patients with precancerous gastric cancer admitted to the Affiliated Hospital of Gansu Medical College from January 2017 to January 2020 were selected for retrospective analysis and divided into two groups according to their surgical treatment plan,with 60 patients in each group.The control group underwent conventional endoscopic submucosal dissection,and the patients in the observation group received painless digestive endoscopic high-frequency electric knife dissection.The resection speed,resection area,curative resection rate,complete and bulk resection rate were compared of the two groups,the patients were followed up for 3 years to record the recurrence rate,and the complications of the two groups of patients were also compared.Results There were no differences in resection area,curative resection rate,complete resection rate and total resection rate between the two groups(P>0.05).The resection speed in the observation group was(27.34±8.58)mm2/min,which was higher than that in the control group(15.54±4.52)mm2/min(P<0.05).There was no difference in the total recurrence rate(28.33% vs 30.00%)at 3 years after surgery between the two groups(P>0.05).The incidence of infection,perforation and other related complications in the observation group was 3.33%,lower than that in the control group(15.00%)(P<0.05).Conclusion sCompared with conventional endoscopic submucosal dissection,high-frequency electric knife under painless digestive endoscopy has significant efficacy in the treatment of gastric precancerous lesions.However,painless endoscopic high-frequency electrosurgical treatment can improve the resection speed and reduce the incidence of complications such as infection,perforation and bleeding.
目的 探讨链霉菌抗生物素蛋白-过氧化物酶(SP)免疫组化染色检测胰岛素样生长因子-1(IGF-1)及组织P53蛋白的表达在结直肠腺癌中意义。方法 选取乌鲁木齐市中医医院2020年1月—2023年12月收治的169例结直肠腺癌患者为恶性组,另选取我院同期收治的169例良性结直肠肿瘤患者为良性组。取手术或活检病理组织应用SP免疫组化染色检测2组肿瘤组织IGF-1及P53蛋白表达水平,对比良性组与恶性组间差异,分析不同临床分期、淋巴结转移情况及组织分化程度患者的IGF-1水平、IGF-1mRNA及P53蛋白阳性率。结果 SP免疫组化染色检测IGF-1阳性率、P53蛋白阳性率,恶性组(72.78%、47.93%)均高于良性组(14.79%、6.51%),对比差异有统计学意义(χ2=115.440、χ2=73.180,P<0.05);Ⅳ期患者IGF-1阳性率(96.77%)及P53蛋白阳性率(77.42%)高于Ⅲ期(85.11%、63.83%)、Ⅱ期(62.69%、31.34%)及Ⅰ期患者(45.83%、25.00%),对比差异有统计学意义(χ2=24.860,χ2=28.000,P<0.05),各亚组两两比较差异无统计学意义(IGF-1阳性与阴性,Ⅰ期 vs Ⅲ期,χ2=12.110,P<0.001;Ⅰ期 vs Ⅳ期,χ2=16.060,P<0.001;Ⅱ期 vs Ⅲ期,χ2=6.880,P=0.009;P53蛋白阳性与阴性Ⅰ期 vs Ⅲ期,χ2=9.580,P<0.002;Ⅰ期 vs Ⅳ期,χ2=14.9990,P<0.001;Ⅱ期 vs Ⅲ期,χ2=11.790,P=0.001;);有淋巴结转移患者IGF-1阳性率(85.71%)及P53蛋白阳性率(71.43%)高于无淋巴结转移患者(14.79%、40.94%),对比差异有统计学意义(χ2=4.720、11.740,P<0.05);低分化患者IGF-1阳性率(93.48%)及P53蛋白阳性率(71.74%)高于中分化患者(81.18%、54.12%)、高分化患者(52.63%、31.58%),对比差异有统计学意义(χ2=21.250、13.510,P<0.05)。结论 SP免疫组化染色检测提示结直肠腺癌患者IGF-1、P53蛋白阳性率高于良性肿瘤患者,且临床分期高、淋巴结转移与肿瘤组织低分化者的IGF-1、P53蛋白阳性率高,因此IGF-1、P53有望成为结直肠腺癌诊治的重要检测指标。
Objective Exploring the significance of immunohistochemical staining with streptavidin-perosidase(SP)to detect the expression of insulin-like growth factor-1(IGF-1)and tissue P53 in colorectal adenocarcinoma.Methods A total of 169 patients with colorectal adenocarcinoma admitted to a hospital from January 2020 to December 2023 were selected and divided into the malignant group.Other 169 patients with benign colorectal tumors admitted to a hospital during the same period were selected and divided into the benign group.SP immunohistochemistry staining was used to detect and compare the expression levels of IGF-1 and P53 proteins in two groups of tumor tissues obtained in surgery or biopsy.And the IGF-1 levels,IGF-1 mRNA,and P53 positivity rates in patients with different clinical stages,lymph node metastasis,and tissue differentiation levels were compared.Results There was a significant difference in the positive rates of IGF-1mRNA and P53 detected by SP immunohistochemistry staining,and malignant group(72.78%,47.93%)were higher than the benign group(14.79%,6.51%),which were statistically significant(χ2=115.440,73.180,P<0.05).The positive rates of IGF-1 and P53 were 96.77% and 77.42% in stage IV,which were higher than those in Stage III(85.11%,63.83%),II(62.69%,31.34%)and I(45.83%,25.00%),the differences were statistically significant(χ2=24.860,28.000,P<0.05).The expression levels of IGF-1 and P53 in colorectal adenocarcinoma patients with different lymph node metastases showed significant differences,the positive rates of IGF-1(85.71%)and P53(71.43%)in patients with lymph node metastasis were higher than those without lymph node metastasis(14.79%,40.94%),and the differences were statistically significant(χ2=4.720,11.740,P<0.05).There were significant differences in the expression levels of IGF-1 and P53 among patients with colorectal adenocarcinoma of different degrees of differentiation,the positive rates of IGF-1(93.48%)and P53(71.74%)were significantly higher than those of moderately differentiated(81.18%,54.12%)and well differentiated(52.63%,31.58%),and the differences were statistically significant(χ2=21.250,13.510,P<0.05).Conclusion sThrough SP immunohistochemical staining,it was found that the positivity rates of IGF-1 and P53 in colorectal adenocarcinoma patients were higher than those in benign tumor patients,and those with high the clinical stage,lymph node metastasis,and low differentiation of tumor tissue,had higher the positivity rates of IGF-1 and P53.Therefore,IGF-1 and P53 are expected to become important monitoring indicators for the diagnosis and treatment of colorectal adenocarcinoma.
目的 探讨动态对比增强磁共振成像(DCE-MRI)多参数定量特征对乳腺癌腋窝淋巴结转移(ALNM)风险的预测价值。方法 回顾性收集2020年3月—2022年11月在佛山市高明区人民医院经手术病理确诊的155例乳腺癌患者临床资料,根据患者是否发生ALNM分为ALNM 组(n=39)和无ALNM 组(n=116)。采用单因素分析和多因素Logistic回归分析乳腺癌发生ALNM的影响因素。结果 ALNM组和无ALNM 组患者的肿块质地、肿块直径、肿块部位、肿块形状、肿块内部强化特征等指标比较差异无统计学意义(t/χ2=2.249、0.977、1.369、0.524、2.158,P>0.05)。两组患者肿块表观扩散系数(ADC)值、腋窝淋巴结(ALN)短径、肿块边缘、动态增强时间-信号强度曲线(TIC)曲线等指标比较,差异有统计学意义(t/χ2=6.573、9.873、29.441、2.031,P<0.05)。二元Logistic回归模型结果显示,肿块ADC值、ALN 短径(≥5 mm)、TIC曲线(流出型)为乳腺癌ALNM发生的危险因素(OR=0.251、0.106、0.002,P<0.05)。结论 DCE-MRI多参数定量特征中,乳腺癌患者的肿块ADC值低、ALN 短径(≥5 mm)、TIC曲线(流出型)为乳腺癌ALNM发生的危险因素。
Objective To investigate the predictive value of multi-parameter quantitative features of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)in the risk of axillary lymph node metastasis(ALNM)in breast cancer.Methods Clinical data of 155 patients with breast cancer diagnosed by surgery and pathology in Foshan Gaoming District People's hospital from March 2020 to November 2022 were retrospectively collected.According to whether the patients had ALNM,they were divided into ALNM group(n=39)and non-ALNM group(n=116).Univariate analysis and multiple Logistic regression models were used to explore the influencing factors of ALNM in breast cancer.Results There was no significant difference in mass texture,mass diameter,mass location,mass shape and internal enhancement between the ALNM group and the non-ALNM group(t/χ2=2.249,0.977,1.369,0.524,2.158,P>0.05).There were significant differences in ADC value,ALN short diameter,tumor margin and TIC curve between the two groups(t/χ2=6.573,9.873,29.441,2.031,P<0.05).Binary Logistic regression model showed that ADC value,ALN short diameter(≥5 mm)and tumor margin(blur)were risk factors for the occurrence of breast cancer ALNM(OR=0.251,0.106,0.002,P<0.05).Conclusions Among the multi-parameter quantitative features of DCE-MRI,the ADC value of breast cancer,the short diameter of ALN(≥5 mm),and the edge of the tumor(blur)are the risk factors for the occurrence of ALNM in breast cancer.
目的 探讨表皮生长因子受体酪氨酸酶抑制剂(EGFR-TKIs)一线治疗耐药后,二线化学治疗(化疗)联合程序性死亡蛋白1及其配体(PD-1/L1)免疫检查点抑制剂方案对晚期非小细胞肺癌(NSCLC)的疗效。方法 选取2018年 6月—2022年10月期间就诊于南通大学附属肿瘤医院院的80例有完整临床资料、应用EGFR-TKIs耐药后晚期NSCLC患者进行回顾性分析,依照不同治疗方式将患者分为观察组与对照组,均为40例。对照组一线EGFR-TKIs治疗耐药后进行二线化疗,观察组一线EGFR-TKIs治疗耐药后进行二线化疗联合PD-1/L1免疫检查点抑制剂治疗。对比两组临床疗效及无进展生存期(PFS),化疗前后血清中人细胞角蛋白21-1片段(Cyfra21-1)、糖类抗原125(CA125)、碱性成纤维细胞生长因子(bFGF)、血管内皮生长因子(VEGF)水平变化,不良反应发生率及生存质量。结果 观察组客观缓解率与疾病控制率高于对照组(P<0.05),对照组PFS为10(2.38,24.13)个月,观察组PFS为14(5.27~,5.27)个月,观察组高于对照组(χ2=4.536,P=0.041);化疗后两组bFGF、VEGF,CA125、Cyfra21-1肿瘤标志物水平均比化疗前降低,且观察组[(17.85±3.32)ng/L、(310.51±88.37)ng/L、(51.62±13.66)U/mL、(10.26±3.37)ng/mL]低于对照组[(19.62±3.24)ng/L、(366.26±49.42)ng/L、(59.26±9.35)U/mL、(12.62±2.73)ng/mL],对比差异有统计学意义(t1=2.413,P1=0.018;t2=3.482,P2<0.001;t3=2.919,P3=0.005;t4=3.442,P4<0.001);两组不良反应发生率对比差异无统计学意义(P>0.05);化疗后两组世界卫生组织生存质量量表简表评分均升高,观察组[(98.62±8.24)、(101.53±12.62)、(95.28±11.15)、(97.79±10.47)分]高于对照组[(84.25±7.32)、(93.58±15.75)、(82.24±9.34)、(83.47±8.38)]分,对比差异有统计学意义(t1=8.246,P1<0.001;t2=2.491,P2=0.015;t3=5.670,P3<0.001;t4=6.753,P4<0.001)。结论 对EGFR-TKIs耐药后晚期非小细胞肺癌患者采取二线化疗联合PD-1/L1免疫检查点抑制剂可提升其临床疗效及生存期,改善血清相关肿瘤标志物表达水平,提升患者生存质量。
Objective To explore the therapeutic effect of second-line chemotherapy combined with PD-1/L1 immune checkpoint inhibitor regimen on advanced non-small cell lung cancer(NSCLC) after epidermal growth factor receptor-tyrosine kinase inhibitors(EGFR-TKIs)resistance in first-line chemotherapy.Methods Retrospectively selected 80 patients with advanced NSCLC EGFR TKIs resistance,who were admitted to the Affiliated Cancer Hospital of Nantong University from June 2018 to October 2022.Patients were divided into an observation group and a control group according to different treatment methods,with 40 cases in each group.The control group received second-line chemotherapy after first-line EGFR-TKIs therapy resistance,while the observation group received second-line chemotherapy and PD-1/L1 inhibitor after first-line EGFR-TKIs therapy reactions and quality of live.Clinical efficacy and PFS,changes in serum levels of human Cyfra21-1,CA125,bFGF,VEGF,incidence of adverse chemotherapy of two groups were compared.Results The ORR and DCR of the observation group were significantly higher than those of the control group(P<0.05).The mean PFS of the control group was 10(2.38-24.13)months,while the mean PFS of the observation group was 14(5.27-35.27)months.The observation group was higher than the control group(χ2=4.536,P=0.041).After chemotherapy,levels of bFGF,VEGF,CA125 and Cyfra21-1 tumor markers decreased in both groups,and the observation group [(17.85±3.32)ng/L,(310.51±88.37)ng/L,(51.62±13.66)U/mL,(10.26±3.37)ng/mL] was lower than the control group [(19.62±3.24)ng/L,(366.26±49.42)ng/L,(59.26±9.35)U/mL,(12.62±2.73)ng/mL],which showed statistically significant difference in the comparison(t1=2.413,P1=0.018;t2=3.482,P2<0.001;t3=2.919,P3=0.005;t4=3.442,P4<0.001).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).After treatment,the WHO QOL-BREF scores increased in both patient groups and the observation group scores[(98.62±8.24),(101.53±12.62),(95.28±11.15),(97.79±10.47)] were higher than the control group scores[(84.25±7.32),(93.58±15.75),(82.24±9.34),(83.47±8.38)],which showed statistically significant difference.(t1=8.246,P1<0.001;t2=2.491,P2=0.015;t3=5.670,P3<0.001;t4=6.753,P4<0.001).Conclusions The combination of second-line chemotherapy with PD-1/L1 immune checkpoint inhibitors can improve the clinical efficacy and survival of advanced NSCLC patients who are resistant to EGFR-TKIs,improve the expression levels of serum related tumor markers,and enhance the quality of life of patients.
乳腺癌是女性最常见的原发恶性肿瘤之一。目前,通过采用综合治疗手段,包括手术、新辅助治疗、辅助放化疗等多种手段,乳腺癌已成为疗效最佳的实体肿瘤之一。其中,新辅助治疗(NAT)包括新辅助化疗、靶向治疗和内分泌治疗,目的是使肿瘤降期、保乳、保腋窝,并可观察药物敏感性,是当前乳腺癌综合治疗中非常重要的组成部分,其治疗疗效对患者手术方式的选择和预后至关重要。尽管病理学评价在评估NAT疗效方面被公认为金标准,但其局限性在于只能通过有创手段在治疗后进行,无法在治疗前对患者做出准确预测。磁共振成像(MRI)作为一项广泛使用的乳腺成像技术,在评估NAT疗效中扮演着关键角色。近年来,人工智能技术,尤其是影像组学(Radiomics)和深度学习(Deep Learning),在医学影像分析领域取得了显著进展。这些技术能够从医学图像中提取大量肉眼难以识别的特征,揭示病变内部的微观结构和生物学行为,全面反映肿瘤的异质性,这不仅有助于临床医生更准确地区分良、恶性肿瘤,还能对恶性肿瘤的预后进行更为精确的评估。本文系统综述了近年来基于MRI的人工智能技术在乳腺癌新辅助治疗疗效评估中的应用研究,旨在促进人工智能在NAT临床实践中的应用和发展,为乳腺癌NAT治疗策略的优化和个性化医疗的实现提供科学依据。
Breast cancer is one of the most common primary malignant tumors in women.Currently,breast cancer has become one of the most effective solid tumors by using comprehensive treatment methods,including surgery,neoadjuvant therapy,adjuvant radiotherapy and chemotherapy.Among them,neoadjuvant therapy(NAT),including neoadjuvant chemotherapy,targeted therapy and endocrine therapy,is a very important part of the current comprehensive treatment of breast cancer.It aims to reduce the tumor stage,preserve the breast,preserve the armpit,and observe the drug sensitivity.Its therapeutic effect is crucial to the choice of surgical methods and prognosis of patients.Although pathological evaluation is recognized as the gold standard in evaluating the response to NAT,its limitation is that it can only be performed after treatment by invasive means,and cannot accurately predict response before treatment.As a widely used breast imaging technology,magnetic resonance imaging(MRI)plays a key role in evaluating the response to NAT.However,traditional MRI evaluation methods are limited by the individual differences of interobserver and the low repeatability of evaluation results,which affects the accuracy of efficacy evaluation to a certain extent.In recent years,artificial intelligence technology,especially radiomics and deep learning,has made significant progress in the field of medical image analysis.These techniques can extract a large number of features that are difficult to be recognized by the naked eye from medical images,reveal the internal microstructure and biological behavior of the lesion,and fully reflect the heterogeneity of the tumor.This not only helps clinicians to distinguish benign and malignant tumors more accurately,but also makes a more accurate assessment of the prognosis of malignant tumors.This article reviews the application and progress of MRI-based artificial intelligence technology in evaluating the response to neoadjuvant therapy for breast cancer in the past five years,aiming to promote the application and development of artificial intelligence in NAT clinical practice,and provide a scientific basis for the optimization of NAT treatment strategy and the realization of personalized medicine for breast cancer.
目的 通过多种生物信息学方法分析MAML1在GC患者中的表达及与临床特征、预后和免疫治疗疗效的相关性。方法 利用TCGA数据库分析胃癌组织与正常胃黏膜组织中的MAML1表达水平;Kaplan-Meier在线工具对胃癌数据集GSE15459进行分析,阐明MAML1与患者临床特征及分期、治疗疗效的相关性;STRING软件预测与MAML1表达相关的基因,并用FUNRICH软件评估其富集的分子生物学功能和信号通路;TIMER和GEPIA数据库探索MAML1表达水平与肿瘤浸润免疫细胞及其相应基因标记集之间的关系。结果 MAML1在GC组织中的表达水平高于正常组织(P<0.001),且其表达水平与III期、有淋巴结转移、无远处转移的患者生存期相关(P<0.05),而与I、II和IV期、无淋巴结转移和有远处转移的患者生存期无相关性(P>0.05)。MAML1的相关作用基因主要分布在细胞核、参与转录调控,并且主要富集在雄激素受体、C-MYB转录因子和HIF-2α转录调控等相关的信号通路。MAML1表达水平与B细胞、CD4+ T细胞、巨噬细胞的表达水平存在正相关关系(P<0.05),但与肿瘤纯度、CD8+ T细胞、中性粒细胞、树突状细胞无相关性(P>0.05)。结论 MAML1有可能成为GC患者较差的临床预后标志物之一,其潜在分子机制可能与转录调控调节肿瘤微环境有关。
Objective To investigate the expression of MAML1 and its relationship with clinical characteristics, prognosis and the efficiency of immunotherapy in patients with GC. Methods MAML1 expression profile was observed by TCGA database. Kaplan-Meier survival analysis was applied to evaluate the correlation between the expression of MAML1 and clinical characteristics, prognosis and treatment efficiency of patients in GSE15459 dataset. MAML1-associated genes were predicted by STRING and were enriched in GO and KEGG by FUNRICH software. The relationship between MAML1 expression and markers of tumor infiltrated cells were explored by TIMER and GEPIA database. Results MAML1 was abnormally upregulated in GC tissues compared to normal gastric tissues (P<0.001). MAML1 expression was significantly associated with the overall survival of patients in stage III, with lymph node metastasis and without distant metastasis (P<0.001). There was no significant difference between MAML1 expression and the overall survival of patients in stage I, II, IV, without lymph node metastasis and with distant metastasis (P>0.05). MAML1-assoicated genes were mainly located at the nucleus, mediating transcriptional regulation and mainly enriching in androgen receptor, C-MYB transcription factor and HIF-2α transcription regulation and other related signaling pathways. MAML1 expression was positively related with the expression of B cell, CD4+ T cell and macrophages (P<0.05), but without significant difference with tumor purity, CD8+ T cell, neutrophils and dendritic cells (P>0.05). Conclusions MAML1 could be used as a marker of clinical prognosis of patients with GC. The potential molecular mechanism might be associated with its function in transcriptional regulation and changes in tumor microenvironment.
